Privacy Policy
VETERINARY CONCIERGE SERVICE AGREEMENT
This Veterinary Concierge Service Agreement ("Agreement") is made and entered into as of the date of signing by and between:
Provider: [Veterinary Concierge Service Name] ("Provider"), located at [Business Address], and
Client: [Client’s Full Name] ("Client"), residing at [Client's Address].
1. SERVICES PROVIDED Provider agrees to offer veterinary concierge services, including but not limited to:

Telehealth consultations
-
In-home veterinary visits (if applicable)
-
Preventive care recommendations
-
Nutritional and supplement guidance
-
Coordination of specialty referrals
-
Other services as agreed upon
2. AVAILABILITY
-
The Provider is available Monday through Friday, 9:00 AM – 6:00 PM EST.

After-hours and weekend access is limited; however, the Provider will make every effort to respond promptly.
3. FEES AND PAYMENT

Client agrees to pay the applicable service fees as outlined in the fee schedule provided by the Provider.

Payment is due at the time of service unless otherwise agreed.

Any additional fees for emergency consultations or extended services will be communicated in advance.
4. LIMITATIONS OF SERVICE

Provider does not replace emergency veterinary care. In case of a life-threatening emergency, the Client should seek immediate care at an emergency veterinary hospital.

Provider may recommend but does not dispense prescription medications. Any necessary prescriptions must be obtained through a licensed veterinarian or pharmacy.
5. CLIENT RESPONSIBILITIES

Client agrees to provide accurate and complete medical history of their pet(s).

Client understands that Provider’s recommendations do not replace regular veterinary care, vaccinations, or legally required treatments.

Client agrees to follow all guidance and recommendations provided by the Provider.
6. CANCELLATION AND TERMINATION

Appointments must be canceled at least 24 hours in advance to avoid a cancellation fee.

Either party may terminate this agreement with written notice. Fees for services rendered before termination remain due.
7. LIABILITY WAIVER

Provider is not liable for any adverse reactions, complications, or outcomes resulting from advice, treatments, or services rendered.

Client agrees to hold the Provider harmless from any claims or damages resulting from the use of services provided.
8. CONFIDENTIALITY

Provider agrees to maintain the confidentiality of Client and pet information, except as required by law or necessary for the pet’s medical care.
9. GOVERNING LAW This Agreement shall be governed by and construed in accordance with the laws of the state in which the Provider operates.
10. ACKNOWLEDGMENT AND ACCEPTANCE By signing below, both parties acknowledge and agree to the terms outlined in this Agreement.
